Has this been facelifted like the Rapid? I just saw one on the road for the first time yesterday, which made me remember that it exists.

yes there is a really light facelift in 2015. I don't think it's necessary to do a new page for it (i don't see the difference with the 2 versions)

I think they changed the headlights. But then, did anything other than the headlights change on the Rapid?

Yes exactly. I think that means the Toledo should also get a facelifted page. There are good pictures available.
